This form is to be completed and submitted PRIOR to any absence from work. In the case of an emergency absence (due to sickness or injury), the form needs to be submitted at your earliest convenience.

Please understand that you still need to communicate with your Program Manager directly, and ensure that they are aware that you are requesting absence from a shift and you will be looking for a cover.

In the case of an emergency where you are giving less than 24 hours’ notice, you need to CALL your Program Manager.
